Health budget in Cuba: efficiency and equity in the face of macroeconomic challenges

Introduction. The core of the sustainability and effectiveness of health systems lies in the health budget. The allocation and management of financial resources determine a country's capacity to respond to the needs of its population, from prevention and primary health care to the treatment of complex diseases, within a complex macroeconomic context. Cuba is part of this reality.
Objective. To provide an updated review of the health budget in Cuba, the main efficiency challenges in its execution, and its impact on equitable access amid macroeconomic challenges.Methods. A literature review of scientific and technical sources was conducted, identifying various sources, including electronic, internet, institutional archives, and library collections. Google Scholar, PubMed, SciELO, Dialnet, Latindex, and the Virtual Health Library (VHL) were used as search engines.
Results. Updated information on the Cuban health budget was obtained. The categories of efficiency and equity are highlighted as conceptual bases for proper budget execution. Conclusions. The systematic analysis showed a comprehensive and critical synthesis of three axes: models, efficiency, management challenges, and equity. These axes are interrelated and create tension in contemporary practice. The approaches were categorized, and their effectiveness in achieving financial sustainability, efficient administration, and social justice was highlighted. The Cuban health budget is characterized as a complex and unique process, associated with categories such as efficiency and equity in a challenging macroeconomic context.
